Acupuncture and naturopathic medicine new patient appointments are 90 min long. When you call to make your appointment, please specify if you are requesting an acupuncture or naturopathic medicine appointment and with which physician.
New Patients are required to fill out the online questionnaires prior to their visit. Online questionnaires are sent to you via our patient portal in ChARM. You will be sent an email prompting you to create a username and password to access this information once your appointment is scheduled.
If you are having difficulty accessing our patient portal, or you would prefer to fill out paper copies of our forms, you can print out our New Patient Packet and Health Information Form and bring it with you to your first appointment.
If you are a returning patient, you can schedule an appointment by calling our office at (206) 588-1227 or by requesting an appointment through the website or through your ChARM Patient Portal log in.
If you wish to cancel or reschedule an appointment, you must contact us at least 24 hours before your scheduled appointment time. If you do not show up to your appointment without notifying us or you call to reschedule your appointment less than 24 hours in advance you may be charged a cancellation fee of up to $150.00 for your missed appointment.
All information that is transmitted between providers and patients is done through the ChARM patient portal.
Once you have been registered as a new patient with us, you will be sent an email with information on how to create a username and password for our patient portal. You may already have a username and password for ChARM if you have seen another provider that uses this same system.
Once you create a username and password, you can access your questionnaires, labs, images, and all communication with your provider through this system.
